The forms on this page are computerized forms. They will open up in Microsoft Word and can be filled out right in Word. 

How to USE a form.  Here's some hints in filling out the forms:

1. Use the TAB key to move through the form.
    

2. Use SHIFT-TAB to move backwards through the form
    

3. For the most part, you do not have to use the Shift key to capitalize words - capitalization is automatically done.
    

4. Also, the date is automatically formatted.  Example:  If you type 4 4 4 (no slashes), it will automatically format to 4/4/04
    

5. When you get to a Yes & No box, press the Space Bar to insert an "X" in the box (You can also type and "X" in the box).
    

6. When you type in the Comments box, don't press the enter key - the line will automatically wrap to the next line below.

How to DOWNLOAD a form, follow these steps

1. Right-click on the link (below)

2. Select Save Target as

3. Choose the location where you want to save the form,

4. Click on Save.

5. Click on Open.

You may print the form and send it as a hard copy to the ABE office,  OR, if you use Outlook Express as your email client, you can email the completed form as an attachment right to the ABE office from Word.

How to send a form using Outlook Express.

Follow these steps to send the form as an attachment from Microsoft Word:

1. Click on the FIle Menu

2. Click on Send to

3. Click on Mail Recipient (as Attachment)...

4. Type in the email address to the ABE office (marshallabe@starpoint.net)

5. Type a note in the body of the email if you wish.

6. Click on the Send button.
